Multiple data breaches were recorded by the HSE involving health care provided across the Shannonside region last year.
New documents reveal that one incident involved a HSE employee at Sligo University Hospital accessing information without permission.
This data was released to journalist Ken Foxe and the righttoknow.ie website and shows the HSE reported more than 2,000 data breaches last year.
In CHO1, which covers County Leitrim, an incident occurred last year where documents were printed to a printer at other HSE location but were subsequently secured by HSE employee.
In CHO8, just one incident relating to primary care took place in the Longford-Westmeath area when a staff member was discarding their PPE equipment, a document was lost on return from home visit.
A number of incidents were reported in County Roscommon, including an email containing personal information of a HSE employee which was disclosed to another HSE employee at Roscommon University Hospital.
The health data of a patient was inadvertently enclosed with a letter that issued to another patient also at RUH.
Incidents were also recorded at Sligo University Hospital including a HSE employee who accessed information in an unauthorised manner.
In the maternity services at the hospital, a patient record was misfiled in another patients file and viewed by third party.
At Portiuncula Hospital, an email containing personal information was sent to 3rd party email address and one incident was also reported at St Loman's in Westmeath when a staff members mobile number retrieved by service user from a bin.
Other incidents notified by the health services including the discovery of patient lists outside a public premises in Donegal and on the grounds of a hospital in Galway.
