Over half of frail patients presenting to a local hospital were discharged directly home following community referral.
The Frailty at the Front Door service, operated by Portiuncula University Hospital in Ballinasloe, identifies and assesses patients presenting with frailty and provides individualised care.
The service was started in September 2021 and has supported approximately 55% of older patients in being discharged directly home following referral to community health services.
The team supports older adults to live and age well by identifying drivers of frailty and by promoting physical and mental health in those at risk of becoming frail.
According to the latest census reports those aged over 85 are the fastest growing age group, and the service says it's important to recognise and address the challenges faced by older people.
