The family of a Longford woman who died due to medical misadventure following treatment at Mullingar Hospital are calling for a public apology from the HSE.
Bryonny Sainsbury passed away in August 2021, days after she was injured by her horse in Longford.
She was originally taken to Midlands Regional Hospital Mullingar before her transport to Beaumont hospital in Dublin days later.
Significant failures were identified in her treatment at Mullingar Hospital, with an inquest finding her death a result of medical misadventure.
A system's analysis review of Bryonny's treatment highlights ten failures in her treatment and has made 15 recommendations for future patient care.
According to the report, their were a number of missed opportunities for healthcare staff to escalate her care.
Issues were also found with the monitoring of Bryonny's sodium levels and the administration of a specific medicine without an anaesthesiologist present.
Bryonny's mother Alison say that HSE apologies from behind closed doors mean nothing to the family.
